To onboard for COVID-19 screening please follow all three steps listed below.

You are required to complete all three steps.

We bill insurance for the visit and the COVID-19 test.

If you have no insurance the out of pocket cost for the visit is $60 and the out of pocket cost for the COVID test is $67.

Turnaround time for the test is currently 4 to 5 days.

If you require a faster turnaround time for the test a smaller lab called Life Hope Labs can turn around the test in about 24 to 48 hours. The cost for this rapid test is $200. There is limited availability for this service.

Important, once you load the OhMD app described below please take a picture of your drivers license and the front and back of your insurance card and upload to OhMD. If you do not upload this information we will need to reschedule your appointment.


Fill out all demographic and paper work online as listed below: 

In order to electronically fill out and send required paperwork in a secure manner, you must first create a user account. Here are the steps:

  1. Click on the following link and follow the instructions below to fill out: Fill The Form
  2. You will be directed to the secure dochub portal.
  3. Click on the “Login” button on the top right of the screen.
  4. You will be instructed to create a sign in. We recommend you sign in by using your email address, but you can also use google and dropbox to sign in.
  5. Once you have created an account, a confirmation email will be sent to the email address your provided. Open the email and click “Confirm my account”.
  6. Now that you have confirmed your account go back to the dochub page and click on the “Login “ button and enter your sign on information to login in.
  7. Once you are logged in, click on the button on the top right side of the screen labeled “Fill a Copy”.
  8. A screen will pop up titled “Make a Copy” and should have the following options marked.
  9. In the “Make a Copy” box click Fill copy.
  10. Select I agree to use electronic records and signatures and then select “Lets do this”.
  11. Scroll through the document and fill out all pertinent information and sign where required.
  12. When you complete the document then click “Finalize”

We use OhMD to communicate by text and to send your COVID-19 testing results in a secure HIPAA compliant manner. Please download the app. Below are instructions on how to download the OhMD app to your smart phone

  1. Download the app by clicking one of the links above
  2. Open the app
  3. Click continue
  4. Enter your email address then click continue
  5. Enter first name and last name (ensure this is the same first and last name we have on file for you as a patient as this is an important link to establish)
  6. Create a password and agree to the terms.
  7. Enter your phone number.
  8. Then click on text me.
  9. Enter the code that is sent to you.
  10. Click patient
  11. Enter date of birth and gender
  12. Click continue
  13. Click on Dr. Edward Espinosa (if you don’t see “Dr. Edward Espinosa” go ahead and search for it on the messages screen)
  14. And send us a test text.
  15. We will respond back to ensure we are able to communicate with you.

All COVID-19 screening will require an initial telemedicine encounter. VSee is a free telemedicine app that you can download to facilitate the call. Please ensure that once downloaded you set the setting to allow use of camera and microphone for the app. Please download the app well in advance of your scheduled telemedicine call. Please click on the “Vsee” link: Go to VSeeIf you have any difficulty loading the app please try 5 minutes later. Intermittently the VSee telemedicine vendor may experience high volumes which can slow down or stop the download process.
